Transfer Your Prescriptions

Prescription drug coverage is one of the many benefits that GHC-SCW offers to members. To find out if prescription drugs are a covered benefit under your plan, use the MyPlanFinder tool refer to locate your GHC-SCW Benefits Summary.

How to use MyPlanFinder

  • Click here to access MyPlanFinder.
  • Current Members, enter your plan number
  • Potential Members, choose a plan type
  • View, print or save a Benefits Summary and Summary of Benefits and Coverage (SBC)

If you’re a new member, with prescriptions from your previous provider, you can check your medications against the GHC-SCW Drug Formulary available through Navitus.  For more information about using the Drug Formulary and GHC Pharmacies, please review our Pharmacy Quick Facts document here.

If the drug is covered, you won't need to do anything. If you don’t find your medication, it may be listed under a different name or a slightly different version which may be covered. You can get help with questions about coverage for specific drugs by calling the Pharmacy Assistance number.

If you choose to use a GHC-SCW facility to fill your prescriptions, drop off your old prescription bottle at any one of our GHC-SCW clinic pharmacies. We will contact your previous pharmacy to transfer the prescription. In some cases, however, we may not be able to do this while you wait.

If you would like to consider using a pharmacy other than a GHC-SCW pharmacy, please log in to your GHCMyChart, select "My Records" and then select "Prescription Benefit Information". 

As a new member to GHC-SCW, your existing prescriptions may be temporarily honored to give you time to make an appointment with your Primary Care Provider (PCP). At your first appointment, your PCP will evaluate your existing prescriptions. He or she may make some changes to maximize your benefit coverage, such as prescribing similar or even more effective drugs that will have lower co-pays for you.

To expedite curbside pickup:
• Please call 1-2 business days in advance for refill requests. Due to increased volume, requests may take longer than usual.
• Call pharmacy and set up payment before driving in.
• Look for designated Curbside Pickup parking stalls and call pharmacy to let us know you have arrived and what stall you are in.
• Stay in your car and a pharmacy associate will bring it to you.​

ScripTalk Now Available At Your GHC Pharmacy

If you or a loved one has trouble reading the fine print of prescription medication labels, then ScripTalk is the solution for you.  This  FREE, easy-to-use service will read all your prescription label information out loud, including drug name, dosage, instructions, warnings, pharmacy information, doctor name, prescription number, date and more. This service is offered to patients who fill their prescriptions at a GHC pharmacy. Call your GHC pharmacy today to learn more and get signed up.
