The "Well Wisconsin" Program

The “Well Wisconsin” program is an employee wellness incentive initiative for employees and their spouses (or life partners) that are employed by The State of Wisconsin (government offices), Department of Employee Trust Funds (ETF), WPEG (local government), and The University of Wisconsin, including graduate students.

New for 2017

GHC-SCW is no longer administering or providing the payment process for this program. The State of Wisconsin (as the sponsoring employer) has partnered with a new vendor, StayWell to provide all aspects of program administration, such as:

  • Tracking of participation  
  • Scheduling for onsite wellness assessments (biometric screenings) at State offices  
  • Health Care Provider Forms (to bring to your doctor) 
  • Online health surveys (health risk questionnaires/assessments) 
  • Other programming
  • Payments

Note about Wellness Assessments (Biometric Screenings)

It is highly encouraged that State of Wisconsin members attend their onsite screenings provided at their employer’s sites. Please see the StayWell website to schedule your screening.

If members must come to a GHC clinic instead, charges may apply.

If members do come to a GHC Clinic, it is the member’s responsibility to get, read, and bring their StayWell Health Care Provider Form with them to clinic. Members can find this form on the StayWell website.
